ADHD Tool Kit CHADIS 0-3 Autism Screening NICU Followup

We are pleased to announce the availability of a new set of screening tools, CHADIS 0-3, that collect routine clinical data such as feeding, crying and sleep relating to the daily functioning of infants and toddlers. These questionnaires also register parental concerns, which can lead to additional queries that gauge their clinical significance. Further, these questionnaires assess stressful life events and child mood/anxiety.

The CHADIS 0-3 questionnaires are based on the Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood: Revised Edition (DC: 0-3R; Zero to Three, 2005) and our adaptation for primary care (Sturner et al., 2007). When used in conjunction with other CHADIS questionnaires on safety, anticipatory guidance, EPSDT and priorities for the visit, clinicians and staff can eliminate the task of “asking” pre-visit questions, and as a result, can more effectively provide the family-focused care preferred by both doctors and families.

For your convenience, the CHADIS 0-3 questionnaires are available as a single questionnaire called CHADIS 0-3: All Domains or individually as follows:

  • CHADIS 0-3: Sleep
  • CHADIS 0-3: Feeding
  • CHADIS 0-3: Regulation
  • CHADIS 0-3: Life Events
  • CHADIS 0-3: Worries and Moods
Please note that while CHADIS: 0-3 was developed by the same expert panel that created DC: 0-3R, the mental health criteria used are still in the process of validation in general pediatric practice. In addition, the CHADIS: 0-3 questionnaires are based solely on parental report, and therefore any provisional mental health diagnoses that result need to be interpreted with clinical judgment and observation.
